Is FSGS CurableFocal Segmental Glomerulosclerosis (FSGS) can result in urinary protein loss and eventually scar formation of renal glomerular filters, leading to permanent kidney disease in children and adults. It affects kidney function by damaging the tiny structures inside the kidneys that filter blood. And the so-called glomerulosclerosis is used to describe the scarring or hardening of small blood vessels in the kidney. As a result, proteins and red blood cells sometimes leak into the urine.

The most common clinical features of FSGS are Nephrotic Syndrome, which is characterized by systemic edema, proteinuria, hypoproteinemia (abnormally low levels of albumin, usually the most abundant protein in the blood), and hyperlipidemia (high cholesterol).

Is FSGS curable?

The natural progression of FSGS varies widely. Treatment can to some extent alleviate the disease, or at least slow down the development of the disease. Most FSGS patients are not cured. Some patients may initially respond well to treatment, but the disease may recur. Others may continue to slowly lose kidney function and eventually become uremic, although the time varies widely, ranging from 2 to 20 years, and more than 50% of patients will become uremic within 6 to 8 years, possibly requiring dialysis or a kidney transplant.

We know that when patients enter remission stage, the protein in their urine goes down, or even goes into partial remission -- that is, their urine protein goes down significantly, and these patients do much better than those who are resistant to treatment.

Are there drugs that can relieve FSGS?

The goal of FSGS treatment is to maintain adequate nutrition, minimize or eliminate proteinuria, and prevent complications from edema, such as dyspnea, anxiety, or excessive sweating. It is important to control hypertension because if left untreated, it can lead to heart disease, stroke, brain damage, kidney damage or hardening of the arteries. Lowering lipid levels is necessary to reduce cardiovascular risk and may delay the progression of kidney disease.

The most important drugs are those that lower blood pressure. For all glomerular disease, it is important to maintain blood pressure between 120 and 125, above 70; 75-80 for diastolic blood pressure. So lowering blood pressure is very important. A hypertensive drug called an ACE inhibitor reduces proteinuria by reducing the stress and blood pressure as blood circulates through the body.

These are the first essential drugs to be used, but some patients need steroids. That is, glucocorticoids such as prednisone or cyclosporine or other drugs that suppress the immune system.
