Why Does Chronic Renal Failure (CRF) Cause Hypertension Easily
Do you know why Chronic Renal Failure (CRF) causes hypertension easily? Since high blood pressure is an independent risk factor for kidney patients, we should know it clearly so as to take corresponding measures to lower it.
The incidence of hypertension in CRF patients ranges from 70% to 80%. Almost all CRF patients that need renal replacement treatment have hypertension. Three-quarters of CRF patients can control hypertension by removing excess extracellular fluid from body by low-salt diet and dialysis. Another quarter of CRF patients can control hypertension by removing excess sodium and water from body by dialysis. The main mechanisms of hypertension are:
(1) Sodium imbalance. It can result in water and sodium retention so as to increase in total extracellular fluid and cardiac output, and then increase in peripheral resistance, which is the primary factor of CRF hypertension. By controlling water and sodium intake, diuretics and dialysis, high blood pressure can be brought down.
(2) The increase of endogenous digitalis-like factors is a compensatory response to sodium retention. It can inhibit Nat-K +-ATPase in renal tubular epithelial cells and reduce sodium reabsorption in the kidney. However, endogenous digitalis-like factors also inhibit the activity of NatK-ATPase in vascular smooth muscle, increase the intracellular sodium level, inhibit the exchange of Nat-ca2, decrease intracellular calcium outflow, and increase the calcium level in vascular smooth muscle cells, resulting in the increase of vascular smooth muscle cells, and the promotion of sensitivity of smooth muscle tension to vasoconstrictor substances.
(3) The disorder of renin-angiotensin aldosterone system accounts for only 5% - 10% of patients with renal failure.
(4) Reduction of antihypertensive substances secreted by kidney, such as PGE2, PGl2, kallikrein and medullary hypotensive lipid. Those substances can not only dilate blood vessels and natriuretic drainage, but also antagonize the effect of renin-angiotensin aldosterone system. Long-term hypertension not only promotes atherosclerosis and damages the heart, but also is an important factor of cerebrovascular accident in CRF patients.
Now you know why CRF cause high blood pressure easily. If you do not want it to affect your prognosis, you should take treatment as early as possible. Besides conventional antihypertensive drugs, such as ACEI or ARBs, you can try systemic Chinese Medicine Treatment, which can help you restore renal function to regulate the blood pressure.
